Project description:
The World Health Organization emphasizes the critical importance of health literacy in empowering individuals to make informed choices and takecontrol of their own health. Policymakers, health organizations, and all key stakeholders who can help improve population health literacy areexpected to strengthen health literacy activities through patient-centred approaches, clear communication, and the provision of accessible healthinformation. Developing and implementing health literacy programs will promote a more active role and responsibility of individuals in accessing andnavigating the health system, facilitate understanding of the system, improve communication within the system, and facilitate access to healthservices for all vulnerable groups equally. The first national survey in the field of health literacy revealed that almost half of the Slovenian populationhas only limited health literacy, and problems can be observed in the assessment and use of health information and its use in everyday life. Dealingwith health literacy is identified as a particularly pressing issue, as most adults have difficulty navigating the complex health care system. Olderadults, those with lower levels of education, and the financially disadvantaged face additional barriers when it comes to navigating the healthsystem.
Evidence-based patient pathways through the health system are a valuable tool for promoting and improving health literacy in the generalpopulation. Because they involve all stakeholders in the process of prevention, diagnosis, treatment and rehabilitation of patients and their relatives,they rationalize the use of resources and promote multidisciplinary treatment with continuous communication between stakeholders in the team andholistic treatment of the patient, they represent a great contribution to healthcare professionals and the healthcare system. Paths of the patientthrough the health care system enable the establishment of effective quality monitoring in health care and enable meaningful and rationaldigitization of the health care system. Therefore, the project activities are aimed at determining the priority of preparing the path of the patientthrough the healthcare system with the five diseases that represent the biggest burden on society during the implementation of the project. Toprepare the pathway, interviews with experts and patients will be conducted, and all existing professional guidelines will be reviewed. Experts in thefield of the selected disease will be involved in the preparation. The implementation of the patient pathway by the health system has a positiveimpact on the economy. By improving the quality and efficiency of resource use through faster diagnosis, more effective treatment, and shorter rehabilitation times, absence due to illness is reduced, which has a significant impact on economic outcomes.
The aim of the project is to design patient pathways through the health system for five different chronic diseases and conditions that can help health professionals provide consistent and quality patient care and enable patients to navigate the health system rationally and efficiently.
Objectives:

4. To carry out a wide dissemination of research results to the widest possible general and professional public and to prepare the basis for the implementation of the prepared plans for the health care system at the national level.
Working packages:
WP1: Identification of five key chronic diseases/syndromes/conditions
Objective: To identify five key chronic diseases/conditions that represent a burden on the Slovenian healthcare system.
WP2: Preparation of a draft of the national path of a patient with a chronic disease/syndrome/condition through the health care system
Objective: Design the optimal path of the patient through the Slovenian healthcare system for five selected chronic diseases/conditions.
WP3: Definition of indicators and evaluation of prepared paths through the healthcare system
Objective: Evaluate the designed path of a patient with a selected chronic disease/condition through the health system and expand the results.
WP4: Dissemination of research results
Objective: Dissemination of research results with the aim of presenting them to the widest possible general, professional and scientific public, and to prepare the basis for the implementation of prepared pathways into the health system at the national level.
